Food Premise Notification Form
Introduction
Under the provisions of the Food Safety Standard 3.2.2 Section 4, a food business (enterprise or activity that involves the handling of food intended for sale, or the sale of food regardless whether the business is of a commercial, charitable, or community nature), must, before commencing any food handling operations, notify the local authority of their presence and the type of food handling occurring within the premises.
The notification requirement seeks minimal information about a food business. The proprietor of a food business must provide:
- contact details for the food business;
- details to identify the nature of the food business; and
- details of the location of all food premises.
Whether you are purchasing an existing food premise within the Town of Bassendean or establishing a brand new food premise, you are required to complete the attached Food Premises Notification Form.
Please note you only need to complete the notification process ONCE unless your details change.
